| GUIDELINES FOR CONTINUOUS SYNOPSIS RECORD (CSR) |
General Information
In response to the amendments of the SOLAS
requirements to enhance maritime security, the Government of Malaysia, through
the Marine Department of Malaysia shall issue the Continuous Synopsis Records
(CSR) to Malaysian Ships. The CSR
certificate must be kept on board the vessel and shall be available for
inspection at all times.
PROVISION
SOLAS 1974, as amended,
Regulation XI-1/5 specifies the requirement for the applicable SOLAS ships to
have Continuous Synopsis Record (CSR) onboard from 1st July 2004.
A CSR is required for
all Malaysian registered vessels with the exception of :

Fees
The fee for a Continuous
Synopsis Record (CSR) is RM 345.00. The fee is as per The Merchant
Shipping (Surveyors of Ships and Radio Surveyors)(Fees)Rules
in force.

HOW
TO APLLY ?
All
applications for CSR shall
be submitted to the Registrar General of Malaysian Ships at the Ship Registration
Unit, Marine Department Head Office, Marine Department
Malaysia.
1. Application for First CSR
Copy of the following documents shall be submitted
with the application:
v Certificate of Classification
v Document of Compliance
v Safety Management Certificate
v International Ship Security Certificate
2. Application for Revised CSR
Copy of the following documents shall be submitted
with the application:
i.
Form 2 (CSR Amendment form)
ii.
Depending on
the CSR information to be amended, the relevant document shall be submitted:
v CSR Amendment Document
v Information No. 7 - Document of Compliance / Safety Management
Certificate
v Information No. 9 - Document of Compliance
v Information No. 10 - Document of Compliance / Safety Management
Certificate
v Information No. 11 - Certificate of Classification
v Information No. 12 - Document of Compliance
v Information No. 13 - Safety Management Certificate
v Information No. 14 - International Ship Security Certificate
For amendments to CSR information no. 4, 5
(Malaysian Port of Registry only) and 6, the ship’s registry information should
be verified first at the relevant Registrar of Malaysian Ships office according
to established procedures.
3.
Application for CSR – Transfer to Malaysian
Flag
Copy of the following documents shall be submitted
with the application:
v Form 2 (CSR Amendment
form)
v Final CSR from the
previous Flag State
v If other CSR
information (except information no. 4, 5 and 6) are also to be amended
concurrently, as
described in paragraph 2(a) shall also be submitted.
4. Application for CSR –
Closure of Registry
Certificate
of Deletion from the previous Registrar of Ships shall be submitted with the
application.
Whilst the department verifies the submitted information,
it is the responsibility of the owner to ensure that all entries made on the
Application Form are correct. Any incorrect entry may cause delay in the
application process and/or resulted in an error in the CSR document.
